Fill out and submit this form to request a new library card. It usually takes only a few days to process — we will email you when your card is ready! For children 13 years and younger, this form must be completed by a parent or guardian.
Every time a child is born in Grantham, or a child under 12 requests a library card, we celebrate them by adding a book in their name to the library collection.
